REQUIRED FORMS/DOCUMENTS
Your manager needs (3) forms in his/her season binder:
(1) Little League Medical Release Form
(2) Parent Code of Conduct
(3) Release Waiver (formerly Model Release form)
If you player is selected to represent the league in All-Stars, he/she will need to submit proofs of residency. The checklist shows which documents qualify, which includes the School Enrollment Form.
