REQUIRED FORMS/DOCUMENTS

For a normal Fall/Spring season, your manager needs (3) forms in their season binder:

1. Little League Medical Release Form
2. Parent Code of Conduct
3. Release Waiver (formerly Model Release form)

If you player is selected to represent the league in Future Stars / All-Stars, they will need to submit proofs of residency. The checklist shows which documents qualify, which includes the School Enrollment Form.
